Help current adults in custody serve their sentence in a residential reentry center to aid in their reintegration into the community.

  • Client Assessment: Administer risk assessments to identify and target the clientโ€™s criminogenic and non-criminogenic needs and develop individualized case plans.
  • Actively participate in peer review team (PRT) meetings by presenting new clientโ€™s assessments and ISP, updates on clients nearing discharge, provide and eliciting peer support for client issues.
  • Client Support: Conduct weekly case management sessions with BOP and USPO clients to review their progress within the reentry center and community.

Aid clients residing in a Connecticut (CT) Department of Corrections (DOC) funded facility until their end-of-sentence.

  • Client Support: Develop and manage personalized treatment plans for clients, facilitating their community re-entry process. Implemented strategies aimed at reducing drug abuse, preventing recidivism, and ensuring a data-driven approach to rehabilitation.
  • Communication and Collaboration: Maintained effective communication with parole officers and state police to coordinate client oversight and responses in the event of client absconding, per USPO directives.
  • Administrative and Reporting: Utilized the Connecticut Department of Correction Prison Data Management System (PDMS) to collect data and prepare detailed monthly reports on clients' progress, including urine analysis and other drug testing measures.
